NOTE: All Banner generated courses, courses that have an assigned CRN number, including XL/cross listed courses, from Fall 2012 to Summer 2017 inclusive will be archived and then fully deleted from Blackboard on the weekend of January 11/12, 2020. Add Personalized Questions to the SFQ


After you have received an email with a subject of "Humber Student Feedback Questionnaire (SFQ) – Please Create Your Additional Questions Today", you can add a maximum of three personalized questions to your course’s student feedback questionnaire. There are two methods of adding personalized questions to the student feedback questionnaire:

  1. Using your Blackboard course
  2. Direct link from the Email

Method 1:

  1. Log in to your Blackboard course
  2. Click on the + button in course menu and choose Tool link:
  3. Type the name as "Instructor SV+QP", change the type to "Instructor SV+QP", make sure "Available to Users" option is unchecked, and click on Submit:
  4. Now, click on the "Instructor SV+QP" link on the course menu:
  5. You will be taken to the Launch LTI Link page. Click on the Launch button:
  6. On the next page, you will see the list of all courses you are teaching for the current term along with the task type available to you. See the screenshot below:
    1. Task Type: You will choose the course link beside Question Personalization to add questions.
    2. End Date: This is the due date to add personalized questions to the SFQ by.
  7. Click on the course link in the Question Personalization row as shown in the image below:
  8. The question personalization page will open up in a new tab for the course you chose. You can add a maximum of three questions:
  9. Click on the Edit button to start adding personalized questions to the survey:
  10. After typing up your personalized questions, be sure to choose the Select button for each question:
  11. Be sure to click on the Save and Submit buttons once you have typed up all of your personalized questions:

Method 2:

  1. The second method doesn't involve Blackboard. To add personalized questions (maximum of three), you will click on the "Create your additional questions" link from the Email message that you received. The content of the email message will look similar to the following:
  2. The link will open up an Internet browser page on your computer with the list of all courses you are teaching for the current term. Click on the course link where you wish to add peronalized questions into and follow steps 8 to 11 from Method 1 above to add peronalized questions to the survey.

Enable the SFQ Tool in Blackboard


You can enable the SFQ tool for your students through your Blackboard course(s) in addition to the automated email message students will receive past after the question personolization due date.

How to Add the SFQ Tool in a Blackboard Course

  1. Log in to your Blackboard course.
  2. Click on the + button in course menu and choose Tool Link:
  3. Type the name as "Student Feedback Questionnaire", change the type to "Student Feedback Questionnaire", make sure "Available to Users" option is selected, and click on Submit:

The Student feedback questionnaire (SFQ) will also be available to students from outside of Blackboard via direct email message to their preferred email address. Students will receive an email message with a direct link to the student feedback questionnaire past after the due date for question personalization. SFQ Communication - Email Reminders


A series of email reminders are sent out to both instructors and students in order to effectively implement the new Student Feedback Questionnaire (SFQ) tool. Here is what the communication process (email reminders) will look like:

 

Question Personalization (QP)

Instructors will receive an initial email a few days prior to the actual launch of SFQ stating that they can access their courses' surveys to add their own personalized questions. The content of the email will look similar to the following:

 

Instructors will also receive a couple of email reminders about adding personalized questions if they won't save and submit surveys with or without personlized questions by the due date indicated in the intital email. The content of the reminder email messages will look similar to the following:

 

Subject Viewing/Response Rate (SV)

After the surveys will go live and students will start submitting surveys, you will receive an intital email stating to review response rate of student submissions. The content of the initial email will look similar to the following:

Email Reminders to Students

The students will receive an intitial email message stating to complete the survey a few weeks prior to the end of the courses they’re registered for. The content of the initital email will look similar to the following:
