You are required to complete your pre-placement medical requirements (immunizations) before being allowed into placement in the following programs:

  • School of Health Sciences
  • Massage Therapy Program
  • School of Community and Social Services
  • All other programs at both Humber College and/or University of Guelph Humber with a pre-placement medical requirement component e.g. Early Childhood Studies, Kinesiology –Guelph/Humber

Please make sure to contact your School/Program in order to receive updated information regarding immunizations requirements for your placement.

Once you have determined that you need to complete pre-placement medical requirements (immunizations), you can either see your own family doctor, a walk in clinic OR come to Health Services to have your pre-placement immunizations completed.

Please note that this process may take several weeks to complete, and the length of time will vary from person to person. It is recommended to complete all immunizations in accordance with your program deadlines.

The Health Centre encourages all students needing to complete pre-placement medical requirements to come in as soon as it is convenient as this process can be time consuming. The Health Centre will NOT be responsible for students missing their placements related to late completion of immunization records.

If you are interested in having your immunization requirements completed at the Health Centre, please speak to the receptionist about this process. Have the following documents, if available, to get started with the process. The receptionist will also discuss any related fees.

o    Student ID card

o    Valid provincial health insurance card

o    Immunization Record (yellow card)

o    Public Health Immunization Record

o    Documentation of any previous blood test, TB Skin test or chest x-ray results  

o    Previously completed immunization records

The cost of having your pre-placement health forms completed at the Health Centre is a $40.00 Initial Review and Assessment Fee plus other Vaccination Cost.

Students in the School of Health Sciences can have their pre-placement medical records completed at The Health Centre. However, the full clearance to pursue placement is given by the Office of Experiential Learning, School of Health Sciences. The Health Centre provides full pre-placement medical (immunization) clearance and issues an Immunization Clearance Card. This card along with copies of completed medical forms are to be submitted to your program/placement coordinator.

Students in the Massage Therapy Program are expected to pay an additional $40.00 fee for a Full Pre-Placement Passport. This fee includes issuance of an immunization clearance card and reviewing the CPR & First Aid Requirements, Vulnerable Sector Police Check and Mask Fit Certification.

Returning Massage Therapy Students are expected to pay $40.00 to renew their Pre-Placement Passport every year.

All fees/charges originating from the pre-placement medical requirement processes will be discussed with a student. Students are expected to sign an invoice and these charges will then be added to their “myhumber” account, coded as a Health Centre Service Fee. From here they can make the necessary payment.

Note: Students who are having pre-placement medical requirements completed for the first time should use the First Year Intake Forms, whilst those renewing/updating their medical requirements should the Renewal Forms.

Note: In order to have these medical forms completed and/or receive an immunization clearance card and/or receive a pre-placement passport, it would be best to book an appointment with the Health Centre, as limited walk-in spots available.
