Enabling the Student Feedback Questionnaire (SFQ) in Blackboard Ultra

What is the Student Feedback Questionnaire (SFQ)?

The Student Feedback Questionnaire (SFQ) invites students to provide anonymous feedback on their learning experience in each course each semester to support our shared commitment to ensuring the quality of our academic programs at Humber. How do I enable the Student Feedback Questionnaire in my Course?

  1. Go to your course where you want to enable the Student Feedback Questionnaire.
  2. Under Books & Tools on the right side of the screen, select View Course and Institution Tools.

  3. Select the plus sign to the left of the Student Feedback Questionnaire.

  4. Ensure that once the Student Feedback Questionnaire is added to the course, that it is made visible to students.
    The drop-down menu under Student Feedback Questionnaire set to 'Visible to Students'.
    NOTE: Since only students are able to see the Student Feedback Questionnaire, the link will not work if it is accessed by an instructor.
