Systems navigators assist patients and clients in managing their care within the health and social services fields, often acting as the primary source of communication between inter-disciplinary teams and individuals. Due to recent changes to the health-care sector in Ontario, skilled systems navigators will be more in demand than ever for their expertise in guiding and advocating for patients, with potential career opportunities in the fields of:

  • child and youth care
  • mental health and addictions
  • nursing
  • occupational/physical therapy
  • senior care
  • social services

After completing the program and work placement, graduates will be prepared to work across various health and community service systems and will have the skills to assess diverse clients’ service needs and develop personalized care plans to ensure the seamless delivery of care.

Work Placement

You will complete 240 hours of work experience which are reflective of your specialization, and learning and professional development goals. The variety and number of health and community service organizations utilizing systems navigators, along with our extensive industry partnerships, will help you to find a placement opportunity that’s right for you.

Courses

Current students, please make sure to review your MyHumber Academic Progress report to confirm which courses you should register into for your Fall semester. Courses with in-person requirements will likely also have online components. Humber may need to change plans for in-person learning, subject to government and public health directives and/or additional health and safety considerations.

Admission Requirements

Applicants must meet all the requirements below in order to meet minimum program eligibility. Meeting minimum eligibility requirements does not guarantee an offer of admission. Admission decisions will be available via MyHumber, or by email and mail. Admission decisions will not be released by phone.

Admission selection is based on the following 3 requirements:

    Academic

    To be eligible for admission, you must possess the following:

    • A bachelor’s degree, advanced diploma or diploma in health, or social and community service, or other health-related or human services field, such as nursing, social work, social service worker, early childhood educator, occupational therapy, physiotherapy or health sciences.  

    Mature Applicants

    Diplomas and Certificates

    An applicant is considered a mature applicant if they have not completed secondary school or other postsecondary school, and will be 19 or older as of the first day of classes. Humber will invite you for testing to demonstrate that you meet program eligibility.

    Degrees

    An applicant is considered a mature applicant if they have not completed secondary school or attended postsecondary studies, and will be 21 or older as of the first day of classes. Mature applicants for degree programs will be required to meet course requirements at the U/M level or equivalent.

    College Transfer Applicants

    Diplomas and Certificates

    An applicant is considered a college transfer applicant if they have completed some or all of a college-level credential. Humber may use a combination of secondary school and/or college courses and grades to determine program eligibility.

    Degrees

    An applicant is considered a college transfer applicant if they have completed some or all of a college-level credential. Humber may use a combination of secondary school and/or college courses and grades to determine program eligibility. Applicants must have an overall minimum grade point average (GPA) of 65 per cent in the program. Applicants are required to disclose and provide academic transcripts for all course work completed at the postsecondary level.

    University Transfer Applicants

The pre-requisite requirements must be fulfilled before the commencement of your first placement. Participation in placements will not be allowed if not all the pre-requisite requirements are met. Due to the variety of venues and possible changes in legislation, the requirements are subject to change.
