Creating a Blackboard Form

What is a Blackboard Form?

A form is a way to collect students’ thoughts and opinions. The Blackboard Form is listed under the Assessment from Create Item menu; however, it is not a graded assessment by default. Instructors can view the results of the form submission from the Gradebook.

 

How do I create a Blackboard Form?

Creating a Form is similar to creating a Test. Learn how to create a Blackboard Form in the video below:

 

Or follow these step-by-step instructions to add a Form into your course:

  1. Add a new item from the content by clicking the + sign, then select “Create” in the menu.The purple '+' icon to add content items to your course that appears above and below other content items.
  2. Select “Form” from the Create Item side menu.
    The Create Item menu's Form item is under Assessment and is highlighted by a red square.
  3. You can add following types of questions.
    • Essay question
    • Likert question
    • Multiple Choice question
    • True/False question
    • Essay, Multiple Choice, and True/False questions are similar to Test questions, but you don’t provide correct answers, points, or feedback options. You can also perform the following operations:
      • “Reuse questions”,
      • “Add text”,
      • “Add local file”,
      • “Add file from Cloud Storage”,
      • and “Add Page Break”.
        The options for a Blackboard form, including essay, likert, multiple choice, true/false questions.

 

You can add any number of questions to a Blackboard Form. The submitted responses can be accessed from the Gradebook as ungradable items. If you wish to grade the Blackboard Form, such as awarding full marks to all who submitted, you can adjust it from the Blackboard Form Settings visible on the right side when editing the form.

 

What is a best practice for using Blackboard Forms?

Use Likert questions! The Likert question type is a commonly used survey question type and only available in Blackboard Forms (not available in Tests or Assignments!) in Blackboard Ultra.
The Form configuration panel, where you can add question text and define the scale and options for a likert question.
To set up a Likert question, provide a question text, define the scale range, and establish two opposing sides of the scale. Optionally, you may add a “not applicable” option to the question.

  • Scale range: You can select between 3 – 7.
  • Option 1: One side of the scale, for example, “Strongly Disagree”.
  • Option [N]: The other side of the scale. [N] is the number you selected as the scale range. This is the other side of the scale, for example, “Strongly Agree”.

Once saved, the question appears as in the image below.
