What is case management?
Case management is a collaborative process between a student, a case manager, and other involved parties (e.g., faculty or college staff) with the intention of supporting the student’s academic and wellbeing goals. A case manager will meet with a student to assess their needs, provide advocacy, relevant resources, and referrals to services. Case managers work with students to strengthen their navigational skills to better understand institutional policies and procedures and to ensure they are aware of the available options and supports on campus and in the community. Where necessary, a case manager may also follow up with the student and/or involved parties as part of a coordinated care approach.
Why might I want to connect with a Care Coordinator?
To tell you a bit more about the Care Coordinator role, we work with students experiencing an array of challenges (e.g., mental health, financial and food insecurity, homelessness, violence, and many more) to strengthen their navigational skills and ensure they are aware of the available options and appropriate supports on- and off-campus. We can assist students in getting connected or staying connected to supports. Our goal is to support students’ overall wellness and academic success at Humber.
Care Coordinator Service Limitations
Care Coordinators provide non-clinical case management support to current students only. The team does not provide counselling services, rental housing assistance, or tuition support but can provide referrals for students seeking these supports.
How to access Care Coordinator services?
To connect with a Care Coordinator, please email: CARE@humber.ca and a case manager will get back to you within 2-3 business days to schedule an appointment. Care Coordinator services are available by phone, online, or in-person Monday to Friday, 8:30am-4:30pm.
How to refer a student? (for Faculty/Staff)
Case management is a voluntary, student-led process, which requires the active engagement of the student. Please speak with the student before referring to a Care Coordinator to ensure that they wish to engage with supports. You may then complete the Online Care Report, being sure to include the student’s full name, student number, and a brief description of the concern.
Consultations for Faculty/Staff are also available. Please contact CARE@humber.ca to arrange a consultation and a team member will respond within 2-3 business days to schedule an appointment.

SWEL’s Recognize, Respond, & Support (RRS) outlines varying indicators that may suggest a learner is experiencing difficulty: - Academic indicators (e.g., repeated absences, missed assignments, deterioration in quality or quantity of work.) - Behavioural and emotional indicators (e.g., more withdrawn or animated than usual, expressions of severe anxiety or irritability.) - Physical indicators (e.g., deterioration in physical appearance, excessive fatigue.)
Incidents of Sexual Violence
Care Coordinators are the first point of contact for sexual violence (SV) disclosures (recent and historical). Care Coordinators provide survivor-centric, trauma-informed support for students who have been impacted by sexual violence and assist accessing academic considerations, reporting options, health services, safety planning, and additional resources at the college and in the community.
Emergency/Crisis Response
Please note that the Care Coordinators do not offer emergency services or crisis response, and operate from Monday to Friday, 8:30AM-4:30 PM with the exception of College closures. A Care Coordinator will contact you within 2-3 business days to respond to your message.
