Accessible Learning Services (ALS) Student Notice to Release Information
All ALS staff are bound by law and professional ethics to safeguard your privacy and the confidentiality of your personal information. We collect and use only the information that is necessary for the provision of services. For more information on how we collect, use and share information within the Student Wellness and Accessibility Centre, please refer to the STUDENT WELLNESS AND ACCESSIBILITY CENTRE (SWAC) CONFIDENTIALITY FORM.
We share some of your information outside of SWAC, as follows:
Faculty, program coordinator/program head, placement coordinator, academic administrators and academic advisors, in order to facilitate your accommodations. We will not disclose the nature of your disability.
Testing Services, in order to implement testing accommodations. Testing Services also maintains strict levels of confidentiality. We will not disclose the nature of your disability.
Peer Assisted Learning Support (includes note-taking services and peer tutoring), in order to indicate your eligibility for note-taking services (if this is one of your required accommodations). We will not disclose the nature of your disability.
Academic and Career Advising Services, in order to provide limited information related to your accommodation or academic needs should you require advising. We will not disclose the nature of your disability.
George Brown College Deaf/Hard of Hearing Services and/or other Deaf/Hard of Hearing service providers, in order to coordinate ASL, or computerized note-taking services for students who are Deaf or Hard of Hearing.
Humber Financial Aid Office or Guelph-Humber Student Financial Services, in order to confirm your eligibility for OSAP disability related funding (BSWD), and to submit BSWD applications and related correspondence. For Guelph-Humber students taking a reduced course load, ALS must provide confirmation that you are eligible for fees billing by course. We will not disclose the nature of your disability.
Registrar’s Office (Humber): For those students eligible for the Tuition Cap program, ALS will share your eligibility for this accommodation with the Registrar’s Office. We will not disclose the nature of your disability. Your identification as a student with a disability will in no way be reflected on your official academic transcript.
Reporting: ALS is required to provide statistical information to the Ministry of Colleges and Universities (MCU) for internal reporting purposes. In order to comply with these reporting requirements, on occasion we need to work with staff members outside of ALS to help us prepare our statistical reports (such as Information Technology staff). These staff may have access to some of your personal information for a short period of time. Any published or released reports will only include group information (not personal information).
Quality Control Audit: ALS files are subject to quality control audits conducted by MCU. If your file is audited, ALS will limit the disclosure to only that information which is relevant.
