Student Injuries at Humber College
If a Humber student is injured on campus:
- Report injury to Health Services on the day of the injury. If it is after-hours please access closest health care location if medical care is required.
- Students must notify their instructor if injury occurred in a program-related injury.
- Fill out an Injury Treatment Report and fax it to 416-675-6320 (North campus) or 416-252-3416 (Lakeshore campus) on the day of the injury.
Student Clinical/Field Placement Injuries
- Health Science Students - If a student is injured while on placement the instructor must:
- Triage the student’s health care needs.
- Access closest health care location.
- Notify the Occupational Health department of agency where injury occurred.
- Fill out a Student Work Clinical/Field Placement Injury Report and fax it to 416-675-6320 on the day of the injury.
- Community and Social Service Students - If a student is injured while on placement the student must:
- Access closest health care location if medical care is required.
- Notify the Occupational Health department or director of agency where injury occurred.
- Notify Humber instructor and placement co-ordinator.
- Fill out a Student Work Placement Injury Report and fax it to 416-252-3416 on the day of the injury.
Employee Workplace Injuries
If a Humber employee is injured on campus:
- Report injury to the health centre on the day of the injury. If it is after-hours please access closest health care location if medical care is required.
- Employees must notify their manager.
- Fill out an Injury Treatment Report and fax it to 416-675-6320 (North campus) or 416-252-3416 (Lakeshore campus) on the day of the injury.
