COVID-19 Vaccination Status Declaration and Return to Campus Acknowledgement

Dear Humber employees,

We are looking forward to welcoming more of you back to our campuses and facilities this fall. As has been the case throughout the pandemic, our top priority is to ensure the health and safety of our community.

In addition to our recently announced COVID-19 vaccination policy, we have many health and safety measures planned for the Fall 2021 term, including that physical distancing requirements and indoor mask-wearing will continue to be in place across our campuses and facilities.

We are pleased to share with you further details of the COVID-19 vaccination policy and Return to Campus Acknowledgement form.

1. Beginning Monday, August 30, all employees and students will receive an email from Humber College via DocuSign containing the Vaccination Status Declaration and Return to Campus Acknowledgement, along with instructions for completing the form. This is a DocuSign form. Please note that you will need an Internet connection and browser to complete the form. You will not need any special software.

2. Employees and students must complete and submit the Return to Campus acknowledgement form by 11:59 p.m. EDT on Monday, September 6, 2021. Employees will be asked to declare either:

   a. that you are fully vaccinated
   b. that you are not yet fully vaccinated and are intending to become fully vaccinated by Monday, October 18, 2021
   c. that you are requesting an exemption

More information on these scenarios is outlined in the chart below. Regardless of your declaration, you will receive a follow up email from Humber confirming the status you submitted and outlining next steps.

If you indicate your status as b) or c), you will be required to provide proof of a current COVID-19 negative test result in order to access campus, until you are fully vaccinated or have received an individualized accommodation plan or until alternate work arrangements are put in place.

All students and employees are required to complete the form. This includes those who may be planning to fully learn and work remotely and are not expecting to come to campus during the fall term.

3. By October 18, all individuals coming to a Humber campus or facility in person must provide proof that they are fully vaccinated or have received an exemption, and that any accommodation(s) granted is being followed. 

4. Details about the process for providing proof of COVID-19 vaccination will be shared with the Humber community by mid-to-late September.

The COVID-19 vaccination policy applies to all students, employees, contractors and visitors to Humber’s campuses or facilities. It supports previously announced requirements for varsity athletes, students living in residence and those involved with programs with specific work-integrated learning experiences that already require staff/students to be fully vaccinated.

A vaccination policy FAQ has been created to help provide further information and it is updated on an ongoing basis. Employees who may have further questions should speak with their manager.
