Please read disclaimer below and fill out form to report a lost or stolen parking permit/gate card.

Parking permits and gate cards remain the property of humber at all times.

In accepting a replacement permit I agree to pay the  $25.00 non refundable service charge.  The missing  permit is declared invalid and all its parking privileges are terminated immediately.

If located I will return the missing permit to parking services without claim to a refund.

I further understand that any use of a permit declared missing is illegal and the permit is subject to confiscation by  parking services.   Any vehicle parked with an illegal permit will be subject to a municipal parking infraction notice.

In accepting a replacement gate card I agree to pay the $25.00 non refundable service charge.  The missing gate card is declared invalid and all its lot access rights are terminated immediately.

If located I will return the missing gate card to parking services without claim to a refund.

I further understand that any use of a gate card declared missing is illegal and the gate card is subject to confiscation by parking services.

The driver of any vehicle with an illegal permit / gate card will be subject to discipline under humber parking regulations and administrative guidelines.
