All tests and exams which are submitted to the Test Centre must be accompanied by a completed referral form.
This referral form provides us with crucial information about the test or exam to ensure that we are following the rules and guidelines of the test/exam as set out by the faculty member. These rules and regulations of the test/exam include:
- The length of time the student is permitted to write the test/exam
- The final date the student has to write the test/exam
- Authorized materials which the student can use to write their test (calculators, open notes, exam booklet, etc.).
The lower half of the sheet allows instructors to fill out the testing accommodations which are relevant to the student, if they are registered with Disability Services and have a Memo to Faculty.
For your convenience, the Test Centre Referral Form can be found on our website at www.humber.ca/test-centre/faculty-guidelines-missed-test or hardcopies of the Referral Form are available in our Testing Centres or Divisional Offices. Gold Forms are for Humber exams, and green forms are for GH exams.
We appreciate all tests/exams being submitted to the Testing Centre at least 2 business days prior to the student test date. Please ensure all tests/exams you are submitting to the Test Centre have a completed Referral Form attached.
