What happens when an irrevocable beneficiary passes away? Can the Plan Member change the beneficiary designation?

In the event that the irrevocable beneficiary is deceased, the employee may change the nomination. At time of claim, we would need a copy of the death certificate of the irrevocable beneficiary and the new nomination form, which has to be completed after the date of death of the irrevocable beneficiary.
